Home Health Nurse
The Challenge
In the field a home health nurse can access and capture information about a client and the visit which unlocks insights that help her deliver better outcomes for her client.
I worked on this project as UI/UX designer along with a lead designer at IBM.

User As Is Scenario
Not being able to find relevant information about my clients easily.
The amount of paperwork; hard to find time to write up notes for each visit.
Difficult to find time to fit in all my work.
Spending too much time taking and reading notes and not enough time with patients.
From Note Book To Tablet
Through the research we discovered that home health nurse often group their clients using their note book. In the app we allowed users to create groups which users found very useful and an improvement over their note book, in this case our user Jill create a group by area of ‘East Longmeadow’
Patient Profile
One are that came up frequently in user interviews was the problem of finding up-to-date information about the patient. The records were a mix of electronic records and notes in files that were hard to find and access as well as being hard to update.
With the profile screen we could solve several user pain points. The nurse can look up the patients Bettys profile with the information she needs for the visit. If Betty has a wearable the data is displayed on her profile with inbuilt monitoring results displayed to help the nurse pinpoint anything to be concerned about. The nurse can also see the medication patient is on which means, some issues can arise when a patient is on several medications which can conflict with each other if the nurse and doctor don’t have access to information on what the patient is currently taking.
Area Insights
Area insights gives the nurse information on demographics on an area that they are entering. This kind of information is important as patterns in peoples living standards can be linked to their health and well being.
Sentiment Analysis
One of the user problems we solved is quick analysis of a patients prior history using tools such as sentiment analysis. A nurse can ask the Ai system about the patients history, the ai uses natural language processing to show and identify relevant results.
Patient Actions
Jill the nurse can then see many more patients while also focusing more on each patient. Access to information and documentation saves Jill time. She can quickly see what tasks and actions are needed to give the best patient care.
